ACORD 84 is the Dwelling Fire Application used to collect applicant, property, coverage, and underwriting information for a dwelling policy submission. Despite the form's name, it can request more than fire coverage. The insurer's quote and policy determine the covered causes of loss, limits, deductibles, conditions, and exclusions.
What is ACORD 84 used for?
Vertafore's current forms map identifies ACORD 84 as the Dwelling Fire Application.[1] An Oklahoma Insurance Department report records ACORD's filing of the revised 2013/09 edition for a dwelling fire program.[2] The application may be used for a tenant occupied rental, seasonal residence, vacant dwelling, owner occupied property, or another dwelling risk, depending on the insurer's program.
What information does the form collect?
ACORD 84 organizes applicant and location information, requested limits, deductibles, optional coverages, and property underwriting details. Vertafore maps dwelling, other structures, personal property, rental value, loss of use, personal liability, medical payments, and several optional endorsements to the form.[3] The application also asks about occupancy, construction, year built, renovations, utilities, heating, roof, protection, prior insurance, and losses.
What should a property owner verify?
State who occupies the dwelling and how it is used. Report vacancy, renovations, short or seasonal occupancy, and rental activity when requested. Confirm the construction type, living area, number of units, year built, and dates of major roof, wiring, plumbing, and heating updates. Describe supplemental heat, fuel tanks, pools, and other features accurately.
Review the requested building amount separately from market value. Land value and local sale prices do not necessarily equal the cost basis an insurer uses to rebuild. Ask how the proposed limit was calculated and whether replacement cost or another loss settlement basis is being quoted.
Does “dwelling fire” mean fire is the only coverage?
Not necessarily. The form can request different coverage forms and endorsements, but a checked box does not guarantee acceptance. The quote, binder, declarations, policy, and endorsements identify covered causes of loss and exclusions. Review wind, water, theft, liability, and vacancy terms instead of relying on the product name.
Where can you obtain ACORD 84?
Eligible users can obtain current forms through ACORD's Forms Portal.[4] Use the edition supplied by the agency or authorized system and attach any requested valuation or supplemental material.
Frequently asked questions
Is ACORD 84 only for rental homes
No. It can support several dwelling uses, subject to an insurer's program. The stated occupancy and use must match the actual property.
Does the requested dwelling limit equal the home's sale price
Not automatically. Ask which valuation and loss settlement method the insurer is using. The issued policy controls how a covered loss is valued.
The Bottom Line
