ACORD 128 is the Garage and Dealers Section used to collect underwriting information for auto dealers, repair shops, service stations, parking operations, and related businesses. It describes the operation, vehicle storage, drivers, dealer inventory, customer autos, and requested coverages. It is an application section, not proof that garage coverage has been issued.

Who uses ACORD 128?

Vertafore's current commercial forms map identifies ACORD 128 as the Garage and Dealers Section and lists the 2015/12 edition.[1] The section can support submissions for franchised or independent dealers, repair facilities, service stations, trailer dealers, storage garages, and parking businesses. The actual insurer may require supplemental questions for specialized operations.

What information does it collect?

The section distinguishes the type of business, locations, vehicle storage, employee counts, remuneration, dealer inventory, and vehicles held for others. It also collects driver and loss information and requested coverage details. Vertafore notes that driver names on ACORD 128 use separate first and last name fields and that additional drivers can flow to ACORD 163.[2]

What should a garage business verify?

Describe every revenue producing operation, including towing, body work, fuel sales, valet work, or overnight storage when applicable. Report how many customer vehicles may be in the business's care at one time and the highest realistic total value, not an average that ignores busy periods. Separate owned inventory from customer autos. Confirm where vehicles are kept, how keys are controlled, and whether lots are fenced, lit, or monitored.

Does ACORD 128 select all coverage terms?

No. It supplies underwriting and requested coverage information. State specific ACORD 138 forms can record local coverage and limit elections. The insurer's quote, binder, declarations, coverage forms, and endorsements establish the final contract. Review them for covered auto symbols, garagekeepers terms, deductibles, limits, and exclusions.

Where can you get ACORD 128?

Eligible users can search the ACORD Forms Portal for the current form.[3] Complete the copy supplied by the agency or authorized system and keep all attached schedules.
