ACORD 141 is the Crime Section of a commercial insurance application. It organizes requested crime coverages and limits with information about employees, assets, financial procedures, money and securities, premises protection, and prior losses. It supports underwriting, but it does not guarantee that every requested crime exposure will be covered.

What is ACORD 141 used for?

Vertafore's current commercial lines map identifies ACORD 141 as the Crime Section and lists the 2016/03 edition.[1] Its September 2026 release notes describe new Crime Coverages and Crime Underwriting pages mapped to that edition.[2] The form is completed for each applicable location and accompanies the commercial applicant information.

What information does the Crime Section collect?

The section records the requested coverage basis, limits, and deductibles for listed crime exposures. It also gathers employee counts, total assets, employee benefit plan information when applicable, money and securities exposures, accounting practices, audit procedures, check controls, computer safeguards, messenger protection, safes, vaults, alarms, and other premises security.

Do not treat similar labels as interchangeable. Employee theft, forgery or alteration, computer fraud, funds transfer fraud, and robbery or burglary address different events and policy requirements. Ask the agency or insurer to explain the proposed forms rather than inferring coverage from the application label.

What should a business verify?

Describe actual procedures, including exceptions. Identify who can initiate and approve payments, who reconciles bank accounts, how often audits occur, and whether duties are divided. If one person performs several functions, state that rather than describing an ideal process. Confirm employee and volunteer counts, asset values, peak amounts at risk, and how money or securities move between locations.

Complete location details separately when protection differs. Verify alarm service, safe or vault details, guards, messengers, and other safeguards against current records. Explain prior crime losses or known incidents as requested.

Does ACORD 141 provide crime coverage?

No. It is an application section. An insurer may request more information, quote selected coverages, change limits or deductibles, add conditions, or decline the risk. The binder, declarations, crime coverage forms, and endorsements show what was issued. Review definitions and exclusions because everyday uses of words such as “fraud” or “theft” may not match policy terms.

Eligible users can get current forms through ACORD's Forms Portal.[3] Use the edition supplied by the agency or authorized system, and keep each location section and supplement with the signed application.
