An ACORD 68 followed by a state abbreviation may be a completely different document.
What choice does the countrywide ACORD 68 record?
The Electronic Delivery Supplement presents separate choices for electronic delivery, electronic plus paper delivery, rejection of electronic delivery, and withdrawal of consent. It identifies the producer, carrier, applicant or named insured, policy number, effective date, and delivery contacts. The 2019/06 specimen shows those choices and fields directly.[1]
Why does the state suffix matter?
ACORD reuses the base number for state-specific forms. For example, the same index identifies ACORD 68 NJ as a New Jersey commercial fire application and ACORD 68 NY as a New York claims-made liability supplement. ACORD 68 TX is a Spanish homeowners and personal-property rights document.[2] Search for the full code printed on the page, including its state and edition.
Does choosing electronic delivery change the policy?
No. It changes the delivery method for the documents identified by the insurer and applicable law. It does not select limits, add coverage, waive exclusions, or replace the policy. Save downloaded policy documents in a durable location and notify the insurer or agency promptly when the delivery address changes.
Where can you get the correct form?
Use the copy supplied by the insurer or authorized agency system. Eligible users can obtain current editions through ACORD's Forms Portal.[3] Do not download a form based only on the number because a state variant may serve a different purpose. For a homeowners application, compare the delivery supplement with the underlying ACORD 80 rather than treating it as application content.
