Why does the form ask about hours and cooking equipment?

Those facts describe both the public foot traffic and the fire exposure. They give the broker a complete baseline submission; individual markets may request documented follow-up details.

Can I start if the restaurant is a new venture?

Yes. The form distinguishes a new venture from an established operation and collects projected sales when prior sales are not available.

Can you write this where my business is?
